Job Description:
As a Home Health Care Community Liaison, you will play a crucial role in connecting our organization with community resources, building relationships with referral sources, and promoting awareness of our services. You will serve as a liaison between our agency and various community organizations, healthcare providers, and potential clients to ensure seamless coordination of care and exceptional service delivery.


Develop and maintain relationships with healthcare professionals, hospitals, nursing homes, and other community organizations to promote our services and increase referrals.
Educate healthcare professionals and community members about the range of home health care services we offer and how they can benefit their patients or clients.
Participate in community events, health fairs, and other networking opportunities to raise awareness of our agency and establish our presence in the community.
Collaborate with our clinical team to ensure smooth transitions of care and continuity of services for our clients.
Conduct assessments of potential clients and their homes to determine their needs and eligibility for our services.
Provide support and assistance to clients and their families in navigating the healthcare system and accessing community resources.

Bachelor's degree in healthcare administration, marketing, communications, or related field preferred.
Previous experience in healthcare sales, marketing, or community outreach preferred.
Strong communication, interpersonal, and networking skills.
Knowledge of home health care services and the healthcare industry.
Ability to work independently and as part of a team, with excellent organizational and time management skills.
Valid driver's license and reliable transportation.
